You are required to report all disabled members, including those on a medical leave of absence, to GroupSource as soon as possible. Your Billing Administrator will move the disabled member to a separate class on your monthly billing statement, and change their status from Active to Leave of Absence. Please review all members not actively at work monthly and notify your Billing Administrator if the employee has returned to work. Having a separate class and status helps GroupSource identify members not actively at work to ensure that these members do not receive adjustments to their pre-disability salary or benefits, if applicable. If the member is off work for 6 months or more due to an injury or illness, they should apply for Waiver of Premiums so that Life & AD&D benefits can be continued on a non-premium paying basis, even if there are no disability benefits on your group plan.
Please review the specific class Booklet(s) to determine if the following benefits are included in your plan before contacting GroupSource to apply for disability benefits. All applications for disability benefits can be sent by email to our Disability Claims Administrators at disability@groupsource.ca.
Short Term Disability
All members who have been absent from work for five consecutive days as a result of an injury, illness or accident should be provided with a Short Term Disability claim form. Complete instructions on how to submit the claim are on the application itself. For confidentiality and privacy purposes, the member can send their portion of the application directly to our Disability Claims Administrators by email at disability@groupsource.ca.
Long Term Disability
Applications for long term disability benefits can be obtained by contacting our Disability Claims Administrators at disability@groupsource.ca.
If the member has been on Short Term Disability benefits, and it is determined that the member is going to be off work longer than the LTD elimination period, a Long Term Disability Benefits Application package will be provided to the member by GroupSource 30 days prior to end of the elimination period. Complete instructions on how to submit the claim are included in this package. The member can send their portion of the application directly to GroupSource.
