In the event of the death of a member or dependent, or diagnosis of a critical illness (if applicable to your plan), special claim forms and other documentation will be required before the claim payment can be processed.
Tip!
In the case of a death claim, you will be required to provide GroupSource the employee’s original signed Enrollment Card. This requirement will be waived if we have an e-signed enrollment card via enrolME.

- Life Insurance
- Optional Life Insurance
- Accident and Serious Illness (ASI)
- Accidental Death, Disease & Dismemberment (AD&D)
- Critical Illness
