Have the member complete a Member Benefits Change Form indicating the member’s new name, effective date, and reason for change. As a Plan Administrator, you can make this change directly in WEBS™, or send the completed form to your Billing Administrator via email, mail, or fax.
The oneCard reflecting the member name change is available for download in WEBS™ once the change is completed.
If the name change is due to a life event such as a marriage or divorce, the effective date of the change will be the effective date of the life event. Be sure to review the eligibility of dependents based on the life event.
